Understanding the Wellness Module

Guide to log mental well-being, monitor daily stress, complete health check-ins and keep your care team connected.

The Wellness Module shifts the focus to you, the caregiver. It offers the tools and support you need to prioritize your own wellbeing, so you have what it takes to keep showing up for the people who count on you.

Wellness Survey

Wellness Surveys are based on an evaluation called the Caregiver Strain Index. This is a 13-question survey, each answered on a five-point scale from Never to Always. The survey is designed to be taken more than once (even weekly) to keep tabs on how you are doing mentally and emotionally as a caregiver.

The Wellness Survey doesn't currently save progress if it is left partway through. If left partway through, the survey will need to be started over from the beginning.

First Wellness Survey

If you haven't taken a Wellness Survey before, the graph in the Wellness Module will read 'No entries for this metric,' as shown in the picture below:

To begin, click Take Your First Wellness Survey. Adjust the blue ball on the slider to select your response to each question. Once you've answered all the questions, click I'm Ready!

Once you finish your Wellness Survey, your score will be added to the graph, along with tailored strategies to help you manage the complexities of caregiving.

Who Has Access to Wellness Surveys

The Wellness Survey is completely private from others in the Care Hub. Privacy is most important and always protected. Unlike most other things in the app, wellness survey results, stress score, and improvement plans are private to you. No one else in the Care Hub can view them.

Improvement Plans

After completing a Wellness Survey, you can add Improvement Plans to help you build on your results. Choose to commit to specific actions with a start date and a target date, built around areas you want to improve.

To add an Improvement Plan follow these steps:

  1. Click the Improvement Plan dropdown.

  2. Select an area of improvement.

  3. Either manually type an Improvement Plan or select a premade plan from dropdown selection.

  4. Select Add.

  5. Enter due date and click Save.

Your improvement plans will also be sent to your support team.

Support Team

Your Support Team is different from your Care Hub. It's a separate group of people you invite specifically to help you manage caregiver stress, and they don't have access to your Care Hub.

To add members to your support team, follow these steps:

  1. Select the Support Team dropdown.

  2. Click View Support Team.

  3. Either click the green icon or Add Member.

  4. Enter support team member's email address and details.

  5. Support team member will need to Accept Invitation from email.

Your Support Team will receive weekly email updates about you, the caregiver. These updates include any changes to your Wellness Survey or Improvement Plan, along with areas where you may need a helping hand.

Your Support Team is separate from your Care Hub. They're people you personally choose to keep updated on your wellness journey, and don't need to be part of your Care Hub at all.
